Jeff Wilson besitzt zwar einen kleinen Zirkus, hat aber auch eine Menge John Rambo wird von seinem alten Freund Col. Trautman aus dem Gefängnis geholt – doch nicht ohne Hintergedanken. Der Einzelkämpfer soll nach Vietnam zurückkehren, wo auch zehn Jahre nach Kriegsende immer noch amerikanische Soldaten gefangen gehalten werden. Rambo soll ein mögliches Gefangenenlager auskundschaften. Doch der Einsatz läuft gar nicht nach Plan und bald findet sich Rambo nur mit einem Messer und der Vietnamesin Co Bao im tiefsten Dschungel wieder um alleine den Kampf zu gewinnen, den Amerika vor einem Jahrzehnt verloren hat.
